How to get a quote for your firm

Your broker can submit a request to quote for the Kentucky Bar Association Health Plan by reaching out directly to the carriers:

• Medical - Please have your broker email the following information to Anthem requesting a quote for the Kentucky Bar Association Health Plan: 
  • Employer Application  
  • Employee Applications; (For 51+ groups - No applications are needed. Please submit the Underwriting Questionnaire and Census)
  • Waivers for employees not electing coverage
  • Most recent UI-3 (Wage & Tax)
  • Current carrier's renewal (if they have received it)
• Dental - Delta Dental of Kentucky - reach out directly to your assigned sales representative
• Vision - VSP - reach out directly to your assigned sales representative
• Group Life and Disability - OneAmerica - Phil Corrao
• COBRA administration - McGregor - Ashley Willis or 859-618-6257

If you need a health insurance broker, simply select a broker from the list. Direct submissions to the carriers will not be accepted.
